tint/transform/utils: HoistToDeclBefore polish

A new HoistToDeclBefore::InsertBefore() overload that takes a statement
builder. Required for supporting multiple clones.

Remove Apply() - it was API smell that wasn't needed.

Spring-clean the implementation
