David Neto 3ac99398b5 spv: Parse OpExtInst for GLSL.std.450
It will always use the "std::glsl" import name.

Remember all the IDs of such imports.

Never add more than one GLSL.std.450 import to the AST.

Also refactor the Assemble test helper into its own file.
