Rewrite the entire crate, add assembler
- 10x faster disassembly performance
- Nearly feature-complete assembler
- `no_std` compatible
- Relicense to MIT/Apache-2.0
- Remove old crates (dol, flow-graph, etc)
- Remove Python bindings (for now, at least)
